Add a Browser source, tick Local file, then pick the widget's .html file. Set the source to 1920 × 1080 and scale it inside your scene — don't shrink the browser source itself or the text goes fuzzy.
On the full desktop overlays the wallpaper is solid, so a camera placed behind them can't show through. Put your camera source on top of the overlay in OBS and line it up with the webcam window — or use the Camera only version, which is fully transparent.

It's usually the channel name. Open the settings page and type your channel exactly as it appears in your Twitch URL, with no @ and no spaces, then rebuild the link.
Yes — use them on as many of your own streams and videos as you like. What you can't do is resell or give away the files themselves.
